Emergency Preparedness Procedures Manual

  • Bomb Threat Checklist for an operator or person receiving a call that threatens the safety of security of the physician´s office practice (Word file).

  • Emergency contact list to be completed by the practice (Word file).

  • The following documents provide recommended guildelines for primary care practices to use during responses to bioterrorism and other public health emergencies (Word files):
  • Reportable Diseases List provided by the Vermont Department of Health (pdf file).
  • Emergency Preparedness Procedures Manual (Green Book): If your practice hasn't
    received the Green Book, contact your local AHEC (Area Health Education Center) office.
