On May 7, 2004, Southern Vermont AHEC and VCHIP held a conference on Adolescent Health and Community Connections: Teaming Up to Apply Strength-Based Interventions
 
 

Practitioners shared what they learned about the asset approach to interviewing and building health care collaborative teams. The asset-based approach was presented by Paula Duncan, MD, Professor of Pediatrics of the University of Vermont College of Medicine in 2000.

About The Presentations:

  1. Risk and Strength Approaches to Youth: As Director of the Vermont Youth Health Initiative (VYHI) , Dr. Duncan leads a project that is testing a primary-care office-based intervention that combines standard screening with an asset-based interview called "6+4". VYHI helps participating practices with their delivery of preventive services and the implementation of screening and referral.

  2. Screening and Assessment in the Health Care Setting: Rapid behavior changes during adolescence require regular screening for health risk behaviors and to provide health guidance. Dr. DiNicola helps participants improve the efficacy of risk behavior screening and associated assessment of adolescents with health care concerns.

  3. Motivational Interviewing and Brief Intervention: Brief intervention of even a single session can decrease alcohol consumption by 20-50 percent, according to evidence compiled over the past 20 years.
    Dr. Kane gives you practical guidance on how to successfully implement a brief intervention for youth who demonstrate at-risk behavior.

  4. The Asset Approach: With many months of application, the VYHI strength based approach to adolescent health is living up to its promise. Four asset questions were added to the pediatric interview to reflect the protective factors that influence health behaviors. Dr. Duncan gives a broader picture of "assets in action".

  5. The Referral Process: Dr. Frankowski elaborates on how relationships are built between primary care practitioners, school nurses and other members of the community health care team.

  6. Confidentiality: Confidentiality laws are not an exact science, but Vermont does have several statutes covering guardianship and custody, drugs and alcohol, and mental health treatment. Dr. Rooney gives you additional facts and insights gained from 13 years of practice in Vermont and as one of the VYHI participating practices.
